Submit Site Request

Request a site by completing this form through Service Now. If you need approval based on the eligibility criteria, please complete this prior to submitting the form for a new site.

Below are the options explained in further detail.

  • Requested for customer information
    If you are submitting this request on behalf of another person who is currently affilated with USC, enter that person’s name within the ‘Requested for’ field to pull in that person’s information. If the person is not found in our system, or is someone external to the USC system, select “External customer”, then enter the person’s information.
  • Requested for
    Your name.
  • Department
    Your department/school.
  • Email
    Your email.
  • Alternate email
    An alternate email for tracking the progress of the new site request.
  • Phone
    The phone number of the primary site administrator for the site.
  • Alternate Phone
    An alternate phone number for tracking the progress of the new site request.
  • Short Description
    A short description (<50 words) of the site request.
    Example: New WordPress site for Department X on Pantheon
  • Hosting Type
    The options available are:
    • WordPress – A generic WordPress site maintained by Pantheon.
    • USC Gateway Upstream – A custom upstream for WordPress developed and maintained by USC ITS. Read more.
    • Drupal – A generic Drupal site maintained by Pantheon.
  • Performance Plan
    Please see current specifications for choosing Basic, Small, Medium, Large, X-Large.
  • Name of Department/School/Organization for the request
    The Department/School/Organization who will be the primary site administrator will be for the site.
  • Site Owner (please provide a USC email address)
    The person who will be the primary site administrator for the site.
  • Request Details
    Please provide your intended URL, anticipated launch date, and any other pertinent information needed.
